Senior College Application Checklist

1) Turn in Authorization to Release Records form signed by both student and parent. This gives DBMHS permission to release your transcript to colleges.

2) Order official SAT and/or ACT score reports to be sent to the colleges you are applying to (www.collegeboard.orgwww.actstudent.org). DBHS is not responsible for this! Please see your counselor to find out if you are eligible for a fee waiver.

3) Ask 2 teachers for letters of recommendation. Ask in person AND request on Naviance. Your School Counselor will also write you one. Fill out the Student Brag Sheet on Naviance.

4) Finalize your list of colleges on Naviance under "Colleges I'm Thinking About". Complete the FERPA form. If you are using the Common Application to apply to any of your colleges, you must match Naviance with your Common App.

5) Complete college applications online. Several colleges use their own university application found on their college website, whereas many other colleges have switched to the Common Application. The Common App saves time, since you don't have to fill out a separate application for every single college you apply to. Several colleges will accept either application and they do not have a preference for one or the other. Be aware of deadlines!

6) After submitted your applications, forward your confirmation emails to your School Counselor. Once your counselorreceives this, they will send your transcripts, letters of recommendation, and supplemental documents to your colleges.

7) Apply for Financial Aid! You will need to complete the FAFSA to be eligible for need-based aid from a college;  https://www.fafsa.ed.gov/ .For more information, go here .

*Check Naviance on a regular basis for scholarship opportunities!

8) Once you have been accepted and have decided which university you would like to attend, you must formally accept an offer of admission and send the deposit. The deadline for seniors to pick their college is May 1st (National College Decision Day)!
